Retail Customer Orders

If your customer is paying by credit card and wants you to place the order, complete this form with detailed information about which products they wish to purchase.   PTI will create a custom invoice for the order and charge your card.  These orders will garner a commission in your ledger to be paid by the 15th of next month.

It is imperative that you fill in ALL customer information so that they received their educational materials along with their product.
